Registration documentation can include, but is not limited to:

• Original Birth Certificate.
• Inoculation Record, including a copy of the latest Physical Examination.
• 3 Proofs of Residency which must include either a Lease, Tax Bill, or Mortgage Statement, and 2 Utility Bills.
• Transfer Card, if available.
